By entering my name below in place of of my signature, I fully acknowledge that I will be acting as an Independent Contractor and in no way considered an employee of The PHOTOSTIX® Corporation. I agree to only present Pre-Approved Advertisements to the public when marketing the PHOTOSTIX® Peel N' Stick Postcard Booklet product and acknowledge that all ads must first be pre-approved in writing and that PHOTOSTIX® advertisements that are published without written concent will result in a loss of my Independent Contractor Affiliate status. I understand that my commissions are based on 10% of the total selling price (including shipping & handling, but excluding sales tax) on each order that enters my Promo-Code number (aka Affiliate ID Number) in the PayPal Order Notes section at the time the order is placed on-line at our Order Page . I understand that Commission Checks are sent out to my address above on the first day of each month from The PHOTOSTIX® Corporation and that once signed up, I'll be able to view my current months commissions earned and YTD (year to date) totals at my affiliate page. The Commissions Earned are as follows; One Booklet Orders = $0.70, Five Booklet Orders = $2.70, Ten Booklet Orders = $4.00, Twenty Booklet Orders = $7.00, Thirty Booklet Orders = $9.50, Sixty Booklet Orders = $17.00, One Hundred Booklet Orders = $25.50. I acknowledge that I will not be considered an active Independent Contractor until I receive a "Welcome Aboard" emailed confirmation from The PHOTOSTIX® Corporation and that this opportunity pertains only to The PHOTOSTIX® Booklet Product and not to any other products or services the PHOTOSTIX® Corporation offers. There are no start-up fees and no monthly membership fees to participate in this program, just a need and desire to earn some additional income each month working from home.
